In the male patient, the treatment of gynecomastia involves reducing the breast size using methods associated with minimal scar formation. I'm of the opinion that, when possible, liposuction, alone should be used. This is, however, infrequently the case, because of the frequent occurrence of excessive glandular breast tissue, which liposuction cannot correct. I then resort to liposuction, plus removal of the glandular tissue using a curved incision along the perimeter of the lower half of the areola. This is ample for most gynecomastia patients.
Rarely, the degree of gynecomastia is such that skin excess is also a problem. In this instance, skin must be removed, in addition to underlying fat and glandular breast tissue. The method of skin tightening can be variable, depending upon each patient's individual condition.
